This behavior and mood tracker can be used to document a variety of targeted behaviors and efficacy of interventions, including environmental modification, pro-active behavior support strategies, and medication.
1. Identify the student and rater (person filling out the form)
2. Write the times or transitions (i.e. Library, lunch, etc.) in the blank boxes above the rating scales.
3. Refer to the key at the top and circle the most appropriate number for each time period.
4. Complete the Weekly Summary at week end- you will be able to identify a pattern of time of day or setting where the student is experiencing the most difficulty with behavior, sensory, or emotional dysregulation.
